Policy: 1. Sara Companions Services Inc. will provide independent contractors and employees with a photo ID badge to ensure that the field staff is properly identified to the client, their family and the general community. Initial here.

2. The field staff must wear or have on their person the company issued photo ID badge when asked by a supervisor of Sara Companion Services Inc, when services are provided outside of the home and when the clients activities of daily living take him/her away from the home. Initial here.

3. Upon resignation or termination, the contractor or employee will return the ID badge along with final notes, paperwork etc. Initial here.

4. Sara Companions Services Inc. reserves the right to withhold final payment for services until all paperwork and the photo ID badge are returned to the agency.

5. Please refer to the Code of Ethics Police and Procedure for specific expectations regarding professional behavior as a contractor or employee of Sara Companions Services Inc.

6. If the ID badge is lost/stolen, there may be a fee for a replacement. Initial here.
