Job Description: Home Health Aide/ Personal Care Aide

Position Summary

The Home Health Aide provides personal care, supportive services, and health-related tasks under the supervision of a Registered Nurse or other authorized health professional, in accordance with the patient’s plan of care and New York State Department of Health (NYSDOH) regulations.

Essential Duties and Responsibilities

  • ~ Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, ambulation, and transfers
    • ~~~ Perform permitted health-related tasks as assigned and trained
    • ~~~ Observe and report changes in patient condition immediately
    • ~~~ Provide companionship and emotional support
    • ~~~ Maintain a clean and safe patient environment
    • ~~~ Follow the individualized Plan of Care at all times
    • ~~~ Accurately document care provided, including Electronic Visit Verification (EVV)
    • ~~~ Protect patient rights, privacy, and confidentiality

Qualifications

  • ~ Current NYS Home Health Aide and/or Personal Care Aide certification
    • ~~~ Ability to read, write, and communicate effectively in English
    • ~~~ Physical ability to perform job duties safely
    • ~~~ Compliance with health screening, background checks, and training requirements
